Vision - How To Use The ProgramStep 1Activate your card by calling: 1-866-207-3340. Have your card ready. The service representative will enter your information into the system. Step 2Call Davis Vision's Customer Service Number: 800-783-3594. The automated system will ask for your "Client Control Number". This four digit number is located on the front of your card as "Vision Code". Enter this number followed by the pound (#) sign. You can use the automated system to obtain basic service pricing information, to locate a network provider in your area and to listen to information on a laser correction service benefit. You can also dial zero to speak with a representative for questions and to make purchase arrangements. The value advantage program entities you to purchase services, such as routine examinations, frames, lenses and contact lenses at discounted prices. To take advantage of this benefit, you must first prepay DV for basic services and then make an appointment with a participating provider. Basic services may be purchased by Visa, Master Card, a personal check or a money order. All co-payments for lenses enhancements, such as tints or coatings are paid directly to the provider at the time of service. Please note: your social security number and description of the service(s) needed will be required. If services are being requested for a dependent, you must also provided their name, date of birth & service(s) needed. Step 3Use the Provider_Directory to locate and then contact a participating provider to schedule an appointment. Step 4Following your examination, you may select eyeglasses from a broad arrangement of styles, colors and sizes. If you have a valid eyeglass prescription, no additional examination will be necessary. However, we suggest you confirm with your provider that the office accepts outside prescriptions. Step 5Your finished eyewear will be returned to the provider’s office for dispensing along with a one-year unconditional warranty against breakage (eyeglasses completely provided by the Davis Vision laboratory only). Step 6 All charges for the examination and eyewear will be applied against the credit card you provided. |
